How does a beneficiary report that a Plan participant has died?

    To report the death to Empower (the Plan’s recordkeeper):

    Please call Empower's beneficiary support line: 866-442-3888, and please have the participant's full name and social security number when you call.

    Empower will require a certified death certificate be submitted to Empower to add a date of death to the Empower account.
