How do I continue coverage in retirement?

Please fill out our Benefits Change Form to let us know your intentions around continuing coverage. See below for specific options for each type of insurance: 

 

Health Insurance: 

Retired Ministers and staff are eligible for the health benefits available under this Plan in either of the following circumstances: 

·         You are under age 65, and you retire from a Subscribing Employer after performing services as a Minister; or 

·         You are age 65 or older, you are enrolled in Medicare Parts A and B, you retire from a Subscribing Employer after performing services in any capacity and you worked at least 750 hours per calendar year for a Subscribing Employer in five of the ten calendar years preceding the year in which you retire. 

 

Dental Insurance: 

Employees who newly retire are eligible to apply to continue their existing dental coverage with our group plan when they meet the following qualifications and act promptly: 

·         Must be at least 62 years of age at retirement, and 

·         Must have worked a minimum of 750 hours for the employer or as a recognized community minister for at least 5 of the past 10 years, and 

·         Must have been enrolled in our group dental plan on the day immediately preceding the date of retirement. 

 

LTD Insurance: is terminated on your retirement date, as reported in the benefits change form. 

  

Life Insurance:  

Life insurance is terminated on your retirement date, as reported on the transition form. If you would like to convert to an individual policy, review the attached Life Conversion documents to make this request, Guardian will handle this process entirely. You have 31 days after your group life insurance ends to do this. You will not be asked for proof that you’re insurable. (Group number: 0533436)
